All requests will be responded to within twenty-four hours, requests received on the weekend will receive a response the next business day. When requested, Environmental Health and Safety will hold your identity confidential.
All workers have the right to raise a safety or health concern without being retaliated against. If you want to report a safety issue anonymously, please use the University of Chicago Accident and Incident Reporting (UCAIR) form. Note, that you cannot report an injury anonymously.
Examples of environmental health and safety concerns include:
- Mildew or mold concerns;
- Indoor air quality complaints;
- Chemical storage, handling, or
emergency procedures; - Policy or regulatory
clarifications; and - Job safety analysis/risk
If your safety concern is related to asbestos abatement or evaluation and inspection of potentially asbestos-containing materials please complete an Asbestos Request Form.
If you would like to request a hot work permit to perform welding, grinding, or open flame soldering, please complete a Hot Work Permit Request.
