Category

Examples

Injury

Any injury requiring a first aid kit, safety shower, eye wash, or other treatment/intervention
Any injury requiring a visit to Occupational Medicine Clinic, Student Health and Counseling Services, Emergency Department, Needle Stick Hotline, etc.

Fire

Any unanticipated fire or discharge of a fire extinguisher
Any call for outside help to contain/control the fire

Biological

Exposure to biological material through a needle stick, cut, or animal bite
Splash of biological material on mucus membrane (eyes, nose, and mouth)
Any other event where someone believes he/she has been exposed to a biologically hazardous material

Chemical

Any (hazardous) chemical exposure to personnel
Any spill requiring the use of a spill kit OR requires assistance to be cleaned up

Laser

Any eye/skin or other injury from a laser
Any unprotected eye exposure to a laser (Class 3b and 4 only)

Radiation

Spill of either a high concentration OR high volume radioactive material
Lost or stolen radioactive material
Radioactive material transported/removed outside of restricted area

Environmental Release

Hazardous chemical or oil/petroleum product enters a drain
Oil spill of any quantity

Equipment

An event that causes damage to equipment

Property Damage

Water damage or wind damage that will interfere with normal operations and requires immediate repair

*Fatality

*Injury requiring medical attention

*Emergency equipment used (eyewash, extinguisher)

*Laboratory exposures -Chemical ‐ Biological ‐ Laser ‐ Radioactive

*Chemical spill or Oil Spill ‐ Hazardous/toxic ‐ Too large for spill kit ‐ > 42 gallons of oil ‐ Entered drain

*Significant property damage (Flooding)

*Odor complaint leading to building evacuation or occupants struck ill
