Overview

The Radiation Safety Forms are available in both Word and PDF formats. The Word files (PC users) and the PDF files contain embedded forms that will enable you to type your information directly on the screen and print the completed form.

 

Software Requirements

To view, complete, and print the PDF files you'll need to download the "free" Acrobat Reader software.  


Note: Acrobat Reader does not allow you to save your completed forms to disk. 

The ability to save completed forms is available with the Adobe Acrobat product software.
